Introduction While quizzing during informed consent for research to ensure understanding has become commonplace, it is unclear whether the quizzing itself is problematic for potential participants. In this study, we address this issue in a multinational HIV prevention research trial enrolling injection drug users in China and Thailand. Methods Enrolment procedures included an informed consent comprehension quiz. An informed consent survey followed. Results 525 participants completed the informed consent survey (Heng County, China=255, Xinjiang, China=229, Chiang Mai, Thailand=41). Mean age was (...) 33 and mean educational level was 8 yrs. While quizzing was felt to be a good way to determine if a person understands the nature of clinical trial participation (97%) and participants did not generally find the quiz to be problematic, minorities of respondents felt pressured (6%); anxious (5%); bored (5%); minded (5%); and did not find the questions easy (13%). In multivariate analysis, lower educational level was associated with not minding the quizzing (6–10 yrs vs 0–5 yrs: OR=0.27, p=0.03; more than 11 yrs vs 0–5 yrs: OR=0.18, p=0.03). There were also site differences (Heng County vs Xinjiang) in feeling anxious (OR=0.07; p=<0.01), not minding (OR=0.26; p=0.03), being bored (OR=0.25; p=0.01) and not finding the questions easy (OR=0.10; p=<0.01). Conclusions Quizzing during the informed consent process can be problematic for a minority of participants. These problems may be associated with the setting in which research takes place and educational level. Further research is needed to develop, test and implement alternative methods of ensuring comprehension of informed consent. Trial Registration clinicaltrials.gov number NCT00270257. (shrink)

As the aging population, including frail older people, continues to grow in Mainland China, quality of life and end-of-life care for frail older people has attracted much attention. Advance care planning is an effective way to improve end-of-life care for people with advanced diseases, and it is widely used in developed countries; however, it is a new concept in Mainland China. The effects of advance care planning and its acceptability in Mainland China are uncertain because of its culture-sensitive characteristics. The (...) objective of this article is to discuss the serious social issue of caring for frail older people and illustrate the possibility of implementing advance care planning in nursing homes in Mainland China through a review of relevant literature, which will focus on legislation, healthcare system engagement, public engagement, and cultural issues. Recommendations to promote and implement advance care planning include choosing nursing homes as a proper setting, establishing an ethical climate, and enhancing public awareness. (shrink)

Cyclin-dependent kinase 5 has been implicated in Alzheimer's disease pathogenesis. Here, we demonstrate that overexpression of p25, an activator of cdk5, led to increased levels of BACE1 mRNA and protein in vitro and in vivo. A p25/cdk5 responsive region containing multiple sites for signal transducer and activator of transcription was identified in the BACE1 promoter. STAT3 interacts with the BACE1 promoter, and p25-overexpressing mice had elevated levels of pSTAT3 and BACE1, whereas cdk5-deficient mice had reduced levels. Furthermore, mice with a (...) targeted mutation in the STAT3 cdk5 responsive site had lower levels of BACE1. Increased BACE levels in p25 overexpressing mice correlated with enhanced amyloidogenic processing that could be reversed by a cdk5 inhibitor. These data demonstrate a pathway by which p25/cdk5 increases the amyloidogenic processing of APP through STAT3-mediated transcriptional control of BACE1 that could have implications for AD pathogenesis. In numerous papers Jaegwon Kim argues that nonreductive materialists (i.e., those philosophers who believe that there are no irreducible non-physical objects in the universe, and yet there are irreducible psychological properties which are indispensable in intentional psychological explanations) face two problems. One is that intentional mental properties are not causally relevant; the other is that explanations appealing to these properties are excluded by explanations appealing to physical, in particular, microphysical, properties.1 The first problem can be called the problem of epiphenomenalism. (...) The second problem is what Kim calls the problem of explanatory/causal exclusion. Epiphenomenalism is not just a problem for nonreductive materialists, but a problem for psychology (as a science of psychological explanation), and for anyone who believes that our thought is the cause of our action. Kim argues that the exclusion problem, on the other hand, is especially a problem for nonreductive materialists. Nonreductive materialists typically buy into the principle of the causal closure of the physical domain, according to which "any physical event that has a cause at time t has a physical cause at t..... [I]f we trace the causal ancestry of a physical event, we need never go outside the physical domain." [Kim 1989b, p. 280] One cannot reject this principle unless one is willing to go back to Cartesian dualism. But the problem is, if all causation is done at the physical level, then there is no causal work left for mental kinds. If mental kinds do not play any causal role, then intentional psychological explanations that appeal to mental properties are not warranted. Therefore, Kim concludes, nonreductive materialism is an untenable position. These tasks represent connected with each other problems: the Problem of Education having the ontology load from the Way's nature (Fatherland-East); the Problem of the Federalism having theontology load of the System (Motherland-West).

Structuralism about mathematical objects and structuralist accounts of physical computation both face indeterminacy objections. For the former, the problem arises for cases such as the complex roots i and \, for which a automorphism can be defined, thus establishing the structural identity of these importantly distinct mathematical objects. In the case of the latter, the problem arises for logical duals such as AND and OR, which have invertible structural profiles :369–400, 2001). This makes their physical implementations indeterminate, in the sense (...) that their structural profiles alone cannot establish whether a given physical component is an AND-gate or an OR-gate. Doherty has recently shown both problems to be analogous, and has argued that computational structuralism is threatened with the absurd conclusion that computational digits might be indiscernible, such that, if structural properties are all that we have to go on, the binary digit 0 must be treated as identical to the binary digit 1. However, we think that a solution to the indiscernibility problem for mathematical structuralists, drawing on the work of David Hilbert, can be adapted for the analogous problem in the computational case, thereby rescuing the structuralist approach to physical computation. Its wisdom and sensitivity make Personal Knowledge required reading for epistemologists. By stressing the active components in scientific knowing--appraisal and commitment--Polanyi shows that knowledge is less "objective," more complex, and more widely distributed in nature than is tacitly supposed by most epistemologies. Knowing implies a foundation in skills, a confidence in one's ability to judge beyond the range of well-formulated rules, and a commitment to the existence of an answer to one's questions before the answer is in sight. Like a (...) Platonic dialogue, this book conveys more than it states, and the broad foundation of insight embodied in the examples would support more conceptual superstructure than Polanyi provides. Although Marx dubbed landowners one of the ‘three great classes’ of modern society, the most prominent Marxian and socialist thinkers of capitalism and land over the past century – from Lefebvre to Massey to Harvey – have implicitly or explicitly argued that landowners are not capitalism’s ‘third class’, and that the social relations of land are marginal or contingent to the mode of production as a whole. Through assessing the work of Marxist geographers, political economists, value-form theorists, and others who (...) have dismissed the class-status of landowners and blurred the line between ground rent and interest, this article argues that the theory of the landowning class is fundamental to the understanding of the totality of capitalist social relations, as well as to developing more incisive analyses of struggles around housing, land, and movement today. Three things make Father Ong's work on the sixteenth-century dialectician Peter Ramus an important contribution to the history of logic and letters. First, he has prudently avoided the temptation to make Ramus a hero or villain and to evaluate his work on its logical merits. His treatment is therefore balanced and well-directed, for Ramus was neither a great thinker nor a great man. Ramus's reforms appear here as epiphenomena of the humanistic reform of pedagogy, and the connection between logic and (...) the demands of the university curriculum thus receives much needed attention. Finally, this book marks one of the first important attempts to apply the contrast between the personal communication through dialogue with the objective, impersonal conveying of information by the written word to the history of philosophy and the interpretation of the Renaissance.--R. The "perspectivism" of the late Dr. McGilvary's 1939 Carus Lectures has two moments. On the one hand it is an examination of the implications of physical relativity for epistemology. In this area McGilvary is at his best--particularly in clearing up misunderstandings of Einstein. But perspectivism is also a revival of the "man the measure" doctrine, and this has less happy results. Viewing the philosopher's task as the making clear of his own postulates, McGilvary is led to beg many of the (...) questions one would like to see him discuss. If a philosophy is put forward postulationally and is to be more than an abstract schema, it must be formulated positively and it must be applied at every opportunity. Taking Locke's epistemology as a starting-point, Maine de Biran elaborated the notions of expectation of resistance and kinaesthetic response into a theory which attempted to account for the origin of our ideas of personal identity and causation. In this clear and intelligent study, Hallie compares Maine de Biran to the British empiricists, finding him most in sympathy with Berkeley; he also assesses the importance and limitations of this internal critique of empiricism for both empiricism and later French philosophy.--R.
